New Patient Registration

Catchment Area

If you are resident within our practice area (please see above) and would like to register with the practice, please speak to one of our receptionists who will explain the procedure and help with the completion of the registration form.

You will also be asked to complete a questionnaire in order to ensure that our computer holds an accurate record of your past medical history. Both forms are available below

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

If you wish, you can opt to have a new patient health check with the healthcare assistant: please ask reception.

Patient Preference of Practitioner

Patients are registered with the practice, not an individual GP. For administrative reasons your medical card will be issued in the name of one of the doctors; however, you can at any time express a preference for a particular doctor, for either all of your medical needs or on a case by case basis.

That said, not all the doctors in the practice provide all services and specific doctors may not be immediately available
